Re-issuance of the COVID-19 vaccination ticket (coupon)

When you need to apply for the re-issuance of your vaccination ticket (coupon)    

・If you have lost or damaged your vaccination ticket (coupon)

・If your address is changed after the ticket (coupon) has been sent out to you

・If the vaccination ticket (coupon) has not arrived by the expected date

How to apply:

There are 4 ways to apply.

(1) By mail

Fill in the necessary information in the “Application form for re-issuance of the vaccination ticket (coupon)”

and send it to the following address with a return-envelope.

(2) By visiting the Call Center

Fill in the necessary information in the “Application form for re-issuance of the vaccination ticket (coupon)”

and submit it at the counter of the following office.
