Vaccination coupons/tickets have been mailed out to individuals of ages 12-18 and 55-59.
*Of those who are 12 years of age, those who were born before August 1, 2009 will be mailed their coupons/tickets first.
Those whose birthday is either on or after August 1, 2009 will receive their vaccination coupons/tickets at the beginning of the following month.
**For example, in the event that the individual’s birthday is on August 15, their vaccination coupons/tickets should arrive on September 1.
For those of ages 12-59 scheduling vaccination appointments, please refer to the chart below to see when scheduling will be available for you.

Both in-person and telephone reception services will be available temporarily on Thursday, July 22 (national holiday) and Friday, July 23 (national holiday). Reception will proceed as usual on Saturday, July 24.

(*2) In accordance with the conditions of the vaccine supply, the schedule will be decided and announced at a later date. Please note that no one will be able to log in to the vaccination reservation website until the registration period begins.

In the case you would like to get vaccinated at one of the mass vaccination sites operated by the Japanese government or Fukui Prefecture, you can apply for vaccination coupons/tickets.
If either of the following applies to you and you have yet to receive vaccination coupons/tickets, please fill in the Application Form to apply for the coupons/tickets.
（1）Those who are registered residents of Fukui City, 18 years old or older, and would like to get vaccinated at one of the vaccination sites operated by the Japanese government or Fukui Prefecture
（2）Those who are registered residents of Fukui City but do not live in Fukui City and are eligible to get vaccinated in the municipality in which you currently live.

“COVID Vaccination Survey Forms for Citizens with Primary Diseases or Underlying Illnesses” have been sent out on June 10, 2021.
Those with primary diseases or underlying illnesses are next in order in terms of vaccination priority behind medical workers and senior citizens. In order to identify who is eligible, Fukui City is currently undergoing an investigation of citizens within the age range of 12–59 via these survey forms.
For those with primary diseases or underlying illnesses that also would like to be vaccinated, please be sure to mail the survey form in the return envelope enclosed with the survey form by Monday, June 21, 2021. Vaccination tickets/coupons are scheduled to be mailed out around the end of July.

We ask those who have yet to schedule an appointment to please do so as soon as possible. For those who have scheduled an appointment only for the 1st dose or those who wish to receive the vaccination before the scheduled date, please consider re-scheduling appointments.
To reschedule an appointment, you must first cancel your current appointments.

Until the end of July, the Group Vaccination locations listed below will be increasing the number of patients they take in.

Vaccination ticket (coupon)
The vaccination ticket (coupon) will be sent out from Tuesday, April 20, 2021 in order of priority determined by the government.
To ensure the smooth implementation of the vaccination process and to avoid close-contact settings at the vaccination site, the vaccination tickets (coupons) will be sent out in stages.
Please understand that the tickets (coupons) are expected to arrive in the mail of all eligible persons any time between 1-3 days from when the tickets (coupons) are sent out.
The tickets (coupons) meant for those between the ages of 16-64 will be sent out when all preparations are complete.

Those of age 60-64・those with underlying health conditions・Elderly care facility workers/staff members
Those of age 60-64, those with underlying health conditions, and elderly care facility workers/staff members are next in line to get vaccinated after senior citizens (65 or older) in order of priority.
To give priority to the people with underlying health conditions, Fukui City will be sending out a questionnaire around June 10 to those from ages 16 to 59 who are registered as a Fukui City resident. The vaccination coupons (tickets) are scheduled to be sent out mid-July.
For now, we are making preparations to send vaccination tickets to those of age 60-64. The process will take time. Please wait patiently.

COVID-19 (Novel Coronavirus) Vaccination
As of April, 2021, the only COVID-19 vaccine approved in Japan is manufactured by Pfizer. In order to get the maximum efficacy out of the vaccine, you should receive the vaccination twice. The second dose should be received three weeks after the first dose.
The vaccination is expected to prevent the onset of COVID-19. It is proved that less people who get vaccinated against COVID-19 develop the disease compared to those who do not get vaccinated.
Pain at the injection site, fatigue, headache, fever, pains in the muscles and joints are reported possible as side effects. Most of the side effects last for a few days after getting vaccinated.
Less common side effects such as anaphylaxis (a serious allergic reaction) have been reported. If you have an anaphylactic reaction, you will be treated immediately.

About the Vaccination
1 Receive a vaccination ticket (coupon) and notice for the COVID-19 vaccination in a white envelope from Fukui City
The envelope will be sent out to your registered address in order of priority by mail.
If you registered to reroute your mail to a designated address, the envelope will be sent to the designated address instead.
2 Make a reservation for vaccination by calling/visiting the Call Center, via online registration
You must schedule appointments for both vaccination dates and vaccination site. You can choose a vaccination site from a list of registered medical institutions in the city or you can choose to receive the vaccine at a group vaccination venue (Fukui City Gymnasium Sub-Arena or Emergency Management & Learning Center). (Click here for details) Be sure to have your vaccination ticket (coupon) on hand. You will need your 10-digit vaccination number to make a reservation.

The Call Center lines may be busy the first few days after the vaccination tickets (coupons) have been sent out. Please schedule an appointment using our vaccination registration website.

3 Please get vaccinated on the day of your vaccination appointment
Please be sure to bring your vaccination ticket (coupon), your notice for the COVID-19 vaccination, and personal identification (driver’s license, health insurance card, etc.) with you to receive your vaccination.
Novel Coronavirus Vaccine, Fukui City Call Center
Fukui City has established a call center specifically made for the novel coronavirus vaccine.
The call center will be able to schedule appointments and answer most questions about the vaccine.

● When will vaccinations start?
Senior citizens will be given the vaccination at either a certified medical institution or in a group vaccination venue. These vaccinations will start mid-May. After that, vaccinations will proceed according to the priority determined by the government.
●When am I expected to receive my vaccination coupons/tickets?
The coupons/tickets have been sent out to the senior citizens.
Coupons/tickets will be sent out to those eligible for the vaccinations in order of priority.
●What should I do if I lose my vaccination ticket (coupon)?
If your vaccination ticket (coupon) is lost, it can be reissued. Please call the Call Center if this is the case.
●I am currently registered as a citizen of another city (Certificate of Residence/juminhyō is registered in a different city), but I currently live in Fukui City due to my school/work being in Fukui City. Can I get vaccinated in Fukui City?
Yes. It is possible to receive the vaccination in Fukui City. For details, please call the Call Center.
● Is it the case that I must schedule an appointment or else I will not be able to get vaccinated?
We are expected to receive a sufficient amount of doses corresponding to the priority determined by the government.Even in the case that all time slots on the day you would like to get vaccinated are taken, please do not panic and make an appointment on the next possible date.
●After I get vaccinated, do I still need to wear a face mask?
Nothing is certain at this point; we currently do not know for sure whether or not you are still able to transmit the virus to others, nor do we fully know about the efficacy of the vaccine.
Please continue to wear a face mask, wash your hands thoroughly, and take any other preventive measures to reduce risks of infection, even after you receive both doses of the vaccine.
Currently, only reservations up until the end of July can be made.
If your first injection is scheduled for either July 11 or later, depending on the date of the first injection, your second injection will be either August 1 or later. You will be able to make your reservation as soon as August reservations open.
August reservations are currently unavailable due to preparation and are to be determined at a later date.
There are some medical institutions/group vaccination sites, which are still available to take reservations in June. Search for your preferred date, and medical institutions/group vaccination sites accepting reservations will appear.

Also, additional reservations from Sunday, June 6 to Sunday, July 11 at Fukui City Gymnasium have become available.
If you have not received the vaccination yet, please choose one of these medical institutions/vaccination sites, make reservations, and get vaccinated as soon as possible.
Reservations until the end of August are now available.
When making reservations in August, please choose medical institutions/group vaccination sites where 2 doses of vaccination are available, and make a reservation for the 1st dose before Tuesday, August 10, so that you may receive 2 doses within August.
